The survey should take about 10-15 minutes and is conducted in line with the Privacy Act. Your responses are confidential, and you won’t be identified in any way.

TERMS & CONDITIONS

• This prize draw is open to any person who has completed the Burnside Village survey.
• Only one draw-entry per person will be accepted each 12 months.
• A simple ID number cross-check will occur to ensure each entry has completed a survey.
• The random prize draw will take place each month after the month finishes.
• Winners will be notified via the contact details provided.
• If attempts to contact a winner are unsuccessful after 7 days, a new winner will be drawn.
• The prize draw consists of a Burnside Village gift card valued at $250 .
• The prizes will be issued by Burnside Village and will need to be collected from the Concierge, during Trading Hours.
• This prize draw will be undertaken by independent research agency Further Insight (http://www.furtherinsight.com.au/) on behalf of Burnside Village.
• Personal information and survey responses will be de-identified (separated) and not stored in the same data file.
• You are welcome to opt out or decline participation at any stage thereafter. Your responses will be used for research purposes only, with all contact details de-identified (separated) from responses, and responses kept confidential.
